Anyone who lives with a cat knows that when it decides you're done, you're done. A notification is easy to ignore. A cat sitting on your document is not.

No. Maramao doesn't ask for Accessibility or Screen Recording, doesn't read what you type and doesn't send anything anywhere.

⌃⌥⌘M starts a session or pauses it. ⌃⌥⌘B starts a break now, ⌃⌥⌘S skips it, ⌃⌥⌘R snoozes it, and ⌃⌥⌘⌫ is the emergency exit.

Yes. A Skip button shows up after a minute, and you can change that in Settings. Strict mode removes it. Snooze pushes the break back a few minutes instead.

It doesn't. While the microphone or camera is in use, or a presentation, video or call app is fullscreen, the break waits a minute at a time, up to 10 minutes by default.

Maramao isn't notarized by Apple yet, so the first launch needs one extra step. Open it once and close the warning with Done. Then go to System Settings → Privacy & Security, scroll down and click Open Anyway next to the Maramao message, and confirm with your password. You only do this once per version.

Quit Maramao from its menu and move it to the Bin. That's it.

Yes. Maramao is open source under the MIT license. The code is on GitHub, and pull requests with new cats are welcome.
